Catastrophic events associated to the surgical treatment of ostium secundum atrial septal defects. Reasons for not underestimating this type of congenital cardiopathy

چکیده
Objective: The present article was motivated by the frustrating experiences with four patients who underwent surgical treatment of ostium secundum atrial septal defect (ASD-II) and who died in extremely dramatic circumstances. Method: This is a retrospective study based on clinical data. The bibliographical research included: paradoxical thromboembolism (cerebral, pulmonary or mesenteric), central nervous system vascular malformations and anomalous vena cava connections. This research was based on possible events, unexpected and catastrophic, that could have directly caused the patients’ deaths. Results: All patients were female. The operations were performed under cardiopulmonary bypasses with less than 20 minutes of aortic cross-clamping. The causes of death were: non-occlusive intestinal ischemia, rupture of a cerebral aneurysm of the anterior communicating artery, cor pulmonale associated with pulmonary arterial hypertension and thromboembolism and probable cerebral thromboembolism in a child that needed to be reoperated to correct anomalous inferior vena cava drainage into the left atrium. Conclusion: Even being technically easy to treat, death due to atrial septal defects in extremely dramatic circumstances is possible, so “Do not underestimate atrial septal defects in congenital heart surgery”. Double-orifice mitral valve and an associated malformation: secundum atrial septal defect.
The authors report a case of double-orifice mitral valve (DOMV) which showed mitral stenosis and mild insufficiency. An associated anomaly was secundum atrial septal defect. DOMV is an unusual congenital heart defect. The occurrence of this anomaly with or without secundum atrial septal defect is very rare. More often it is associated with other congenital malformations arising from atrioventri...
متن کاملSurgical Treatment of Amplatzer Embolus in a Secundum Atrial Septal Defect Patient
A secundum atrial septal defect is the most common congenital heart defect. Transcatheter treatment of secundum atrial septal defects is a popular and less invasive alternative to surgery. Procedural complications may occur in a wide spectrum, particularly device embolus as the most emergent one, but luckily they do not commonly occur in the clinical setting. Mortality from adverse events relat...
متن کاملSecond-degree Atrioventricular (Mobitz 1) Heart Block in a 52-hour-old Newborn: A Very Rare Case Report
Background: Cardiac conduction disorders are rare syndromes in neonates and children. According to the literature atrial septal defect, especially ostium secundum type, is associated with atrioventricular (AV) block. Wenckebach conduction heart block is very rare in a neonate; however, there is a dearth of research on the prevalence of this type of heart block regarding the neonates and pediatr...
متن کاملAtrial Septal Defects – Clinical Manifestations, Echo Assessment, and Intervention
Atrial septal defect (ASD) is a common congenital abnormality that occurs in the form of ostium secundum, ostium primum, sinus venosus, and rarely, coronary sinus defects. Pathophysiologic consequences of ASDs typically begin in adulthood, and include arrhythmia, paradoxical embolism, cerebral abscess, pulmonary hypertension, and right ventricular failure. Two-dimensional (2D) transthoracic ech...
متن کاملRight atrial function with speckle tracking echocardiography: Do we really need it?
Atrial septal defect (ASD) is one of the most common congenital cardiac anomalies presenting in adulthood. ASD is characterized by a defect in the interatrial septum that allows pulmonary venous return from the left atrium to pass directly to the right atrium. The magnitude of the left-toright shunt across the ASD depends on the defect size, the relative compliance of the ventricles, and the re...
